Before → impact(s) → after
Describe each vehicle’s location, direction, lane, and maneuver before the crash; the harmful events and their order; and post-impact movement and final rest. Use assigned vehicle/person numbers consistently. Include relevant roadway/control conditions and identify the basis for conclusions. Keep the agency report number and HSMV report number consistent on every page.
Attribute the information
Separate what you observed from what a driver or witness reported and what you concluded. Explain evidence such as damage compatibility, debris, tire marks, video, or measurements. Record relevant discrepancies, injuries/complaints, EMS response, citations, towing, and pending follow-up. Avoid unsupported phrases such as failed to maintain control without the underlying facts.
Fictional rear-end example · adapt to actual evidence
Vehicle 2 was stopped eastbound in the through lane at a red signal. Vehicle 1 was traveling eastbound behind Vehicle 2. Driver 1 stated that he did not stop before contact. I observed damage to Vehicle 1’s front and Vehicle 2’s rear, with debris near the reported contact location. Vehicle 1’s front struck Vehicle 2’s rear. Both vehicles came to rest in the eastbound lane. Driver 2 reported neck pain; EMS evaluated Driver 2. No second impact was identified. The first harmful event was collision with a motor vehicle in transport; manner of collision was front-to-rear.
